New Patients 

Being well-prepared for your appointment will ensure that we have the information to provide the best possible care for you. Please take time to complete all New Patient forms and our patient care coordinator will call you to schedule your first appointment!

*HIPAA (Notice of Privacy Practices)

At your first visit, we will complete a Comprehensive Oral Examination which includes a periodontal evaluation, oral cancer screening, chart past dental treatment completed, gather medical and dental history, address any concerns you may have and take necessary radiographs.

Please call and request from your previous dentist, to forward any current bitewing x-rays, full mouth x-rays and/or panoramic radiograph to our email: – [email protected].

 Please bring a current identification card and insurance card if applicable.



Scheduling Policy

Appointment/Cancelation/Missed Appointment Policies 

 Our practice reserves appointment times exclusively for each patient to ensure high-quality, timely dental care. When an appointment is canceled with insufficient notice, it becomes difficult for us to offer that time to another patient in need and is financially detrimental to our practice.

Required Notice

If you need to change or ccel your appointment, we require at least 48 hours’ advance notice. Phone messages/text messages and phones are not monitored during nonbusiness hours. This requirement assists us in effectively running our dental practice. 

 Cancellation / Missed Appointment Charge of $50.00 

 Cancellations with less than 48 hours’ notice, or appointments missed will be documented as a late cancellation/ missed appointment. You will be charged a $50.00 cancellation/missed appointment charge to your account and will need to be paid prior to scheduling any future appointments in our office. Please note that insurance does not cover fees or deposits associated with missed or late-canceled appointments.

One-Time Courtesy Waiver 

 Each patient is allowed one (1) courtesy waiver for a late cancellation or missed appointment.

Medical Emergency Exception

We understand that true medical emergencies can occur unexpectedly. If a patient experiences a medical emergency that prevents them from attending the appointment, the late cancellation/missed appointment charge may be excused at the practice’s discretion.

Multiple Late Cancellations/Missed Appointments 

 Patients with repeated late cancellations or missed appointments may be asked to prepay for future visits or may be limited in scheduling options.

 APPOINTMENT COMMUNICATIONS

Ad Astra utilizes electronic communications to assist us in reminding our patients of their upcoming appointments. We are excited when you share your email address and/or cell phone number with us, so we can communicate electronically! 


Insurance and Financial Information  

All patients are responsible for the estimated portion of treatment fees at the time services are provided. This estimated portion is based on information supplied by your insurance company and is not a guarantee of payment.  Dental insurance is a contract between you and your insurance company and as a courtesy to our patients,  Ad Astra will be happy to file your dental insurance claim on your behalf. 

 Ad Astra accepts cash, check, Visa, Mastercard, Discover and Care Credit. Patients are responsible for all costs of dental treatment. If your insurance company pays more than expected and an overpayment is created on your account, our office will issue a refund to the original method of payment. 

    Balances over 60 days may accumulate a monthly interest rate of 1.5% (annual percentage rate of 18%). In the event of default, you will be responsible for any collection charges and/or attorney fees.
