New Patients 

Being well-prepared for your appointment will ensure that we have the information to provide the best possible care for you. Please take time to complete all New Patient forms and our patient care coordinator will call you to schedule your first appointment!

At your first visit, we will complete a Comprehensive Oral Examination which includes a periodontal evaluation, oral cancer screening, chart past dental treatment completed, gather medical and dental history, address any concerns you may have and take necessary radiographs.

Please call and request from your previous dentist, to forward any current bitewing x-rays, full mouth x-rays and/or panoramic radiograph to our email: – [email protected].

 Please bring a current identification card (& insurance card if applicable).



Scheduling Policy

Appointments in our office are scheduled exclusively for you! We are pleased to reserve appointment times that are convenient for you and your family. In order to provide all patients with times that are convenient for their schedules and to effectively run our dental practice, we require a  48 hour notice of cancellation. If you are not able to make your scheduled appointment time, please contact our office as soon as possible to determine if we have another time that day that may work better with your schedule. 

 Ad Astra utilizes electronic communications to assist us in reminding our patients of their upcoming appointments. We are excited when you share your email address and/or cell phone number with us, so we can communicate electronically! We also confirm appointments by phone if you prefer.

Dental insurance is a contract between you and your insurance company. Our team is very knowledgeable with regards to coverage profiles, but it is impossible for us to keep up with the ever- changing industry of dental insurance. It is very important for you, as the patient, to understand your insurance coverage.

Co-pays are due on the day your treatment is rendered.  Co-pays are an estimate of your total treatment cost and your insurance coverage. 

 As a courtesy to our patients, Ad Astra will be happy to file your dental insurance claim on your behalf.  Please provide an updated insurance card with group and subscriber information to verify eligibility and to correctly submit your claim.

Ad Astra accepts cash, check, Visa, Mastercard, Discover and Care Credit. If utilizing a verified insurance benefit, co-pay and deductible are due on the day your treatment is rendered. Patients are responsible for all costs of dental treatment.
